• Building Use Form Necessary to secure GYM/FIELD email 
• Request to hold Complete BEFORE Camp
• Camp reconciliation
• Camp insurance waiver
• Permission to ride in private vehicle

TRAVEL FORMS 

• Overnight Travel Check List - Check List for coaches/sponsors per District Guidelines WHAT DO YOU NEED
• Request for Travel – this needs to be completed and APPROVED prior to travel along with an itinerary and list of all participants

• Domestic Overnight Trip – ALL athletes/participants need to complete

• Bus Request - request 2 weeks prior and indicate whether Jeffco or Charter* (all charters need to be approved by Brad Horner)
